About This Role

As a Discharge & Patient Flow Specialist, you will play a vital role in ensuring the smooth and efficient movement of patients through our healthcare system. Your expertise will be instrumental in coordinating care transitions, optimizing bed utilization, and enhancing patient satisfaction.

Key Responsibilities

  • Collaborate with healthcare professionals to develop individualized discharge plans for patients.
  • Coordinate communication and information sharing among various departments involved in patient care.
  • Monitor patient flow within the hospital, identifying potential bottlenecks and implementing solutions to improve efficiency.
  • Advocate for patients' needs and ensure their timely access to appropriate resources and services.
  • Contribute to quality improvement initiatives aimed at enhancing patient outcomes and satisfaction.

Required Qualifications & Skills

  • Current registration with the Health Professions Council (HPC).
  • Diploma or Degree in Occupational Therapy.
  • Experience working within an NHS setting.
  • Strong clinical reasoning and decision-making abilities.
  • Excellent communication, interpersonal, and organizational skills.
