Doylestown Health is seeking a highly skilled Patient Care Coordinator to join our team. As a key member of our healthcare team, you will be responsible for ensuring the highest level of care for our patients.
Key Responsibilities- Implement and maintain quality standards and guidelines to achieve excellence in patient care.
- Identify, educate, and support team members on proper documentation processes to ensure accurate and timely care.
- Coordinate optimal transitions of care through effective communication with patients, caregivers, clinicians, and facilities.
- Communicate with and schedule appointments according to care standards, identifying and closing care gaps, and providing support and education on self-management of chronic illnesses.
- Facilitate team meetings with physicians and clinical staff to discuss high-risk patients and enhance the role of medical assistants and other clinical staff.
- Education: High school diploma, medical assistant diploma from an accredited vocational institution, or a community college course in medical assisting.
- Experience: Minimum 3 years of recent experience working in a medical facility as a medical assistant, with primary care office experience preferred.
- Skills: Microsoft Office experience, current health records with appropriate immunizations, and knowledge of healthcare field and medical office protocols/procedures.
- Current medical assistant certificate
- Knowledge of specific assisting tasks related to particular medical practice
- Knowledge of information that must be conveyed to patients and families
