Key Responsibilities:- Implement and maintain quality standards and guidelines to achieve recognition and benchmark primary care.
- Identify, educate, and support team members on proper documentation processes and quality recognition.
- Coordinate optimal transitions of care through communication with patients, caregivers, clinicians, and facilities.
- Communicate with and schedule appointments according to care standards, identify and close care gaps, and provide support and education on self-management of chronic illnesses.
- Facilitate team meetings with physicians and clinical staff to discuss high-risk patients and enhance the role of medical assistants and other clinical staff.
- High school diploma or equivalent, medical assistant diploma from an accredited vocational institution, or community college course in medical assisting.
- Minimum 3 years of recent experience working in a medical facility as a medical assistant, primary care office experience preferred.
- Microsoft Office experience required, current health records with appropriate immunizations to work in the healthcare field.
- Knowledge of healthcare field and medical office protocols/procedures.
- Knowledge of specific assisting tasks related to particular medical practice.
- Knowledge of information that must be conveyed to patients and families.
- Communication skills, tact and diplomacy in interpersonal interactions, understanding patient education needs, and effectively sharing information with patients and families.
- Ability to learn and retain information regarding patient care procedures.
- Ability to project a pleasant and professional image.
- Ability to plan, prioritize, and complete delegated tasks.
- Ability to demonstrate compassion and caring in dealing with others.
