Job Summary

As a Practice Nurse at Primary Care Management Solutions Ltd, you will be responsible for delivering high-quality nursing services to our patients. This role requires a skilled and experienced nurse who can work effectively within a multidisciplinary team to provide care to patients with a range of health needs.

Main Responsibilities

  • Develop and implement health promotion and well-being programmes to support patients in maintaining good health.
  • Implement and evaluate individual treatment plans for patients with chronic diseases, working closely with GPs and other healthcare professionals.
  • Identify and manage patients at risk of developing long-term conditions, providing support and guidance to help them prevent adverse effects on their health.
  • Provide routine nursing care to patients, including wound care, travel medicine, and chronic disease management.
  • Work collaboratively with the clinical team to ensure the safe and effective delivery of patient care.

Person Specification

  • Registered Nurse with a valid registration with the NMC.
  • Postgraduate diploma or degree in Primary Care or a related field (desirable).
  • Experience working in a primary care environment, with a focus on chronic disease management.
  • Excellent clinical knowledge and skills, including wound care, ECGs, venepuncture, and travel medicine.
  • Ability to work effectively within a team, with excellent communication and interpersonal skills.
