About the Role

We are seeking a skilled and compassionate Positive Intervention Practitioner/Social Care Specialist to join our team at Choice Care. As a key member of our care team, you will play a vital role in providing high-quality support to individuals with learning disabilities, autism, and mental health conditions.

Key Responsibilities
  • Deliver personalized care and support to residents, including bathing, toileting, dressing, and feeding, in accordance with their individual care plans.
  • Collaborate with colleagues to develop and implement effective care plans, ensuring the highest standards of Positive Behaviour Support.
  • Participate in ongoing training and professional development to enhance your skills and knowledge in recovery-based approaches and support for individuals with complex needs.
  • Conduct regular behaviour monitoring and contribute to the development of interventions aimed at reducing challenging behaviours.
  • Provide individual and group support to residents, promoting their independence and well-being.
  • Facilitate the implementation of care plan systems within our homes, ensuring seamless communication and collaboration among team members.
