Job Overview

We are seeking an experienced Registered Mental Health Nurse to join our Community Mental Health Team (CMHT) in Berkshire. As a Senior Mental Health Practitioner, you will work with older adults with mental health conditions and co-existing physical health concerns.

Main Responsibilities
  • Undertake comprehensive assessments in various settings, considering the individual's physical, psychological, social, and spiritual needs.
  • Develop and implement care plans in collaboration with service users, families, and carers.
  • Monitor and evaluate care plans, making adjustments as necessary, and reporting at review or CPA meetings.
  • Manage a caseload, working collaboratively with other disciplines from statutory and voluntary sectors.
  • Respond to acute psychiatric problems and take new referrals.
  • Utilize therapeutic interventions, including short-term problem-solving, crisis intervention, and basic CBT techniques.
  • Contribute to service development, participating in projects and plans aimed at improving and maintaining quality of care.

Requirements
  • Registered Mental Health Nurse or equivalent.
  • Two years post-qualification experience.
