Job Overview

Berkshire Healthcare NHS Foundation Trust is seeking a skilled Mental Health Practitioner to join their Older Peoples Mental Health Service team. As a key member of the team, you will work collaboratively with other disciplines to provide high-quality care to patients in the community.

Main Responsibilities

  • Manage a caseload of patients and participate in multi-disciplinary team meetings
  • Undertake assessments of patients and develop care plans in consultation with the patient and the team
  • Supervise Support Workers and/or students
  • Provide advice to other members of the service and agencies on the needs of service users

Requirements

  • Registered Mental Health Nurse or equivalent qualification
  • Experience of working with older people with mental health problems
  • Ability to drive and have a car for use at work
