Positive Intervention Practitioner/Social Care Specialist

We are seeking a skilled and compassionate Positive Intervention Practitioner/Social Care Specialist to join our team at Choice Care. As a key member of our care team, you will play a vital role in providing high-quality support to individuals with learning disabilities, autism, and mental health conditions.
Key Responsibilities- Deliver personalized care and support to residents, including bathing, toileting, dressing, and feeding, in accordance with their individual care plans.
- Work collaboratively with colleagues to develop and implement effective care plans, ensuring the highest standards of Positive Behaviour Support.
- Participate in ongoing training and professional development to enhance your skills and knowledge in areas such as recovery-based approaches and support for people with learning disabilities and/or mental health difficulties.
- Contribute to the development and implementation of interventions aimed at reducing challenging behaviors, and provide individual and group work to residents under supervision.
- Facilitate the implementation of care plan systems within our homes, ensuring seamless communication and collaboration between team members.
