We are seeking a dedicated and compassionate Positive Intervention Practitioner/Social Care Worker to join our team at Choice Care. As a key member of our team, you will play a vital role in providing high-quality care and support to individuals with learning disabilities, autism, and mental health conditions.

About the Role

As a Positive Intervention Practitioner/Social Care Worker, you will be responsible for:

  • Providing personal care, including bathing, toileting, dressing, and feeding
  • Following care plan guidance and company policies and procedures
  • Attending and completing training as required
  • Upholding high standards of Positive Behaviour Support
  • Developing and maintaining a special interest in recovery-based approaches and support for individuals with learning disabilities and/or mental health difficulties
  • Visiting and coordinating ongoing behaviour monitoring of challenging behaviours
  • Assisting in the development of interventions aimed at reducing challenging behaviours
  • Offering individual and group work to individuals receiving services under supervision
  • Facilitating the implementation of Care Plan systems within the homes
