Job Description

As a Care Practitioner at Choice Care, you will play a vital role in providing high-quality care to individuals with learning disabilities and mental health conditions. Your primary responsibility will be to deliver personalized care plans, ensuring the well-being and dignity of our residents.

Key Responsibilities:
  • Implement care plans and provide personal care, including bathing, toileting, and dressing
  • Follow company policies and procedures, and attend training sessions as required
  • Uphold high standards of Positive Behaviour Support and develop a special interest in recovery-based approaches
  • Visit and coordinate ongoing behaviour monitoring, and assist in the development of interventions to reduce challenging behaviours
  • Offer individual and group work to people receiving services under supervision, and facilitate the implementation of Care Plan systems within the homes
Requirements:
  • Be patient, reliable, flexible, and a good communicator
  • Hold Learning Academy - Adult Care Worker Level 2 and Lead Adult Care Worker Level 3 qualifications
