Job Title: Positive Intervention Practitioner/Social Care Worker

Choice Care Group is seeking a dedicated and compassionate Positive Intervention Practitioner/Social Care Worker to join our team. As a key member of our care team, you will provide high-quality care and support to individuals with learning disabilities and/or mental health difficulties.

Key Responsibilities:
  • Provide personal care, including bathing, toileting, dressing, and assistance with feeding
  • Follow care plan guidance and company policies and procedures
  • Attend and complete training as required
  • Uphold high standards of Positive Behaviour Support in line with policy and practice
  • Develop and maintain a special interest in recovery-based approaches and support for individuals with learning disabilities and/or mental health difficulties
  • Co-ordinate ongoing behaviour monitoring of challenging behaviours in relevant Choice Care assigned services
  • Assist in the development of interventions aimed to reduce the intensity, frequency, and potential challenge of behaviours that are challenging to the individual and the service
  • Offer clearly specified individual and group work to people who receive Choice Care services under supervision
  • Facilitate the smooth and efficient implementation of the Care Plan systems within the homes
