About the RoleThis challenging and rewarding role will involve working with vulnerable adults in their own homes, providing personalized support and rehabilitation programs to help them regain independence and confidence. As a key member of our team, you will be responsible for:
- Delivering safe and effective person-centered support to residents, including personal care, meal preparation, and medication management.
- Collaborating with our multidisciplinary team, including Occupational Therapists and Physiotherapists, to develop and implement tailored rehabilitation plans.
- Providing detailed feedback and updates to ensure seamless service delivery and ongoing progress monitoring.
- Liaising with healthcare professionals, families, and carers to ensure comprehensive support and care coordination.
To be successful in this role, you will require:
- A full UK driving license and willingness to use your own vehicle for work purposes.
- The ability to undertake the Care Certificate and maintain up-to-date knowledge of best practices.
- Previous experience working with vulnerable adults, preferably in a community-based setting.
We offer a competitive salary of £25,000 - £30,000 per annum, depending on experience, plus benefits including:
