About the Role

We are seeking a highly skilled and experienced Patient Safety Investigation Lead to join our forward-thinking Patient Safety Team at the North East Ambulance Service NHS Foundation Trust.

Key Responsibilities
  • Oversee the implementation of the National Patient Safety Incident Response Framework, ensuring compliance with regulatory requirements and industry best practices.
  • Lead the analysis and formulation of learning outcomes from patient safety incidents, identifying areas for improvement and developing strategies for mitigation.
  • Coordinate the sharing of lessons learned from patient safety investigations, complaints, and concerns, both internally and externally, to promote a culture of safety and quality.
  • Ensure the completion of Learning From Patient Safety Events (LFPSE) records, monitoring trends in reported patient safety incidents and concerns.
  • Represent the Patient Safety Team, leading effective, timely, and proportionate responses to patient safety incidents, working collaboratively with NEAS staff in accordance with Trust, National, and Commissioner policies and good practice.

Requirements
  • Significant experience in human factors/PSIRF, with a strong understanding of clinical governance and patient safety principles.
  • Education to degree level in a relevant subject, with evidence of continuous professional development at postgraduate level.
  • Teaching/mentorship qualification or demonstrable experience, with a commitment to the development of patient safety and clinical governance.
  • Accredited leadership program or willingness to work towards, with excellent communication, presentational, training, and report writing skills.
