Patient Safety Investigation Lead

We are seeking a highly skilled and experienced Patient Safety Investigation Lead to join our forward-thinking Patient Safety Team at the North East Ambulance Service NHS Foundation Trust.
Key Responsibilities- Oversee the implementation of the National Patient Safety Incident Response Framework, ensuring compliance with regulatory requirements and industry best practices.
- Lead the coordination of patient safety investigations, analyzing incidents and formulating learning outcomes to drive improvement and reduce risk.
- Develop and maintain effective mechanisms for sharing lessons learned from patient safety events, both internally and externally, to promote a culture of safety and transparency.
- Collaborate with cross-functional teams to identify trends and areas for improvement, driving proactive measures to mitigate risks and enhance patient care.
- Provide expert guidance and support to staff on patient safety incident management, LFPSE reporting, and CNST/NHSLA requirements.
- Develop and deliver training programs to enhance staff knowledge and skills in patient safety, human factors, and quality improvement.
- Contribute to the development and implementation of policies and procedures related to patient safety, ensuring alignment with Trust and national standards.

Requirements- Significant experience in human factors and patient safety, with a strong understanding of regulatory requirements and industry best practices.
- Education to degree level in a relevant subject, with evidence of continuous professional development at postgraduate level.
- Teaching and mentorship qualifications or demonstrable experience, with a strong ability to communicate complex information effectively.
- Accredited leadership programs or willingness to work towards, with a proven track record of leading change management projects.
