Job Overview

We are seeking a highly motivated and enthusiastic Rehabilitation Assistant to join our team at Epsom and St Helier University Hospitals NHS Trust. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality patient care and rehabilitation, enabling patients to achieve their therapy goals and promote independence prior to discharge home.

Main Duties of the Job

The successful applicant will work alongside physiotherapists, occupational therapists, nurses, pharmacists, and a GP to deliver patient-centered care and rehabilitation. Key responsibilities will include carrying out therapy plans as per OT/PT recommendations, working as part of the MDT to ensure patients' safe and timely discharge home, and contributing to the development of individualized care plans.

  • Strong communication and interpersonal skills
  • Experience of working autonomously and as part of a team within health or social care
  • Background experience in patient documentation and care planning
  • Basic IT skills, including Microsoft Office and email

Education/Qualification

  • NVQ Level 3 health-related study or equivalent experience
  • Written, verbal, and numeric skills to GCSE or similar level

Experience

  • Experience of a caring role within health or social care
  • Experience of working as a member of a team within health or social care
  • Experience in patient documentation and care planning

Skills/Abilities/Knowledge

  • Demonstrate ability and willingness to learn new skills
  • Basic IT skills, including Microsoft Office and email
  • Demonstrate understanding of the role of a Rehabilitation Assistant
