Position Overview

Epsom and St Helier University Hospitals NHS Trust is dedicated to enhancing patient care through our Reablement Unit, which focuses on facilitating patient discharge and reducing hospital stay durations. The successful candidate will collaborate within a cohesive multidisciplinary team comprising healthcare professionals such as nurses, occupational therapists, physiotherapists, social workers, and rehabilitation assistants. This role may involve working in various settings, including community-based care and acute medical environments.

Key Responsibilities

  • Assist in identifying the health and social care requirements of patients, contributing to the delivery and assessment of rehabilitation services under the guidance of registered health or social care professionals.
  • Collaborate effectively within the multidisciplinary team to foster strong working relationships with statutory, voluntary, and independent organizations.
  • Maintain and enhance clinical skills by engaging with both acute and community rehabilitation services.
  • Provide mentorship and evaluation for junior staff, playing a role in the evolution of this position within the trust and the Reablement Unit.

Job Requirements

Experience

  • Proven experience in therapeutic roles within health or social care settings.
  • Demonstrated ability to work collaboratively as part of a multidisciplinary team.
  • Experience in rehabilitation, whether in inpatient or community contexts.

Skills and Competencies

  • Strong organizational skills with the ability to manage workload independently.
  • Effective time management capabilities.
  • Basic clinical reasoning and problem-solving skills.
  • Proficient in IT and word processing applications.
