Pharmacist Clinical Lead

We are seeking a highly skilled and experienced pharmacist to join our team as a specialist in clinical pharmacy services. The successful candidate will work closely with our dedicated pharmacy team to provide expert advice and guidance on the care and treatment of patients.
Key Responsibilities- Lead Clinical Pharmacy Services: The post holder will lead and manage clinical pharmacy services in the Care Group, working closely with the Principal Pharmacist to ensure that pharmacy services are tailored to meet the needs of the Care Group.
- Provide Expert Advice: The post holder will provide highly specialized and complex evidence-based clinical advice and information to pharmacy, nursing, medical, and other healthcare professionals on the care and treatment of patients.
- Participate in Multidisciplinary Meetings: The post holder will actively participate in multidisciplinary ward rounds, antibiotic ward rounds, and other multidisciplinary meetings to deliver and promote high-quality, cost-effective, patient-focused pharmaceutical care.
- Practice as an Independent Prescriber: The post holder will practice as an independent prescriber, providing direct high-quality care to inpatients and outpatients within the scope of practice.
- Ensure Appropriate Medication Supply: The post holder will ensure the appropriate and timely supply of medication to patients in conjunction with dispensary staff, including the POD scheme and discharge facilitation for patients.
- MPharm
- Registered pharmacist
- Postgrad diploma or equivalent
- Experience of MDT working
