Position Overview
The ideal candidate will be a seasoned clinical pharmacist possessing robust management and leadership capabilities. They will leverage their experience in general practice to enhance the quality and safety of care provided to patients within the primary care network. This role involves leading a team of pharmacists and collaborating with primary care networks to execute various clinical initiatives as directed by the leadership.
Key Responsibilities
The Lead Clinical Pharmacist will guide their Clinical Pharmacy team in developing and managing medication optimization services across network practices. They will ensure the team meets the following objectives:
- Minimize the medication-related workload for physicians.
- Enhance the safety and quality of prescribed medications.
- Offer support on medication-related matters to the entire practice team.
- Drive quality improvement in medication management and prescribing throughout the primary care network.
Additionally, the candidate will support various practice development initiatives, including:
- Focusing on Quality and Outcomes Framework (QoF) domains and improving disease registers.
- Ensuring prescribing safety, conducting clinical audits, and supporting local enhanced services.
- Leading prescription management, including systems and processes, and ensuring proper monitoring.
- Facilitating the delivery of network-wide projects as agreed upon by leadership.
Main Duties
The Lead Clinical Pharmacist will have the following primary duties and areas of responsibility:
- Act as the line manager for clinical pharmacists within the network.
- Introduce new clinical pharmacists to their roles, manage expectations, and outline progression opportunities.
- Oversee the daily management of clinical pharmacists.
- Work alongside clinical pharmacists to address knowledge gaps, demonstrate best practices, and update development plans.
- Organize debrief sessions during team meetings.
- Maintain and enhance professional competence by staying updated with medical and therapeutic advancements, as well as local and national policies.
- Provide ongoing support and management for the pharmacy team.
- Review team progress and clinical work, developing clear plans to achieve results aligned with priorities set by clinical directors.
- Collaborate with practices to implement network-wide projects as directed by leadership.
Training and Supervision
The Lead Clinical Pharmacist will also be responsible for:
- Supervising the development and progression of clinical pharmacists.
- Implementing changes in clinical practice in accordance with their development.
- Leading the appraisal process for clinical pharmacists.
Collaborative Relationships
This role requires effective collaboration with various stakeholders, including:
- Patients, general practitioners, nurses, and other practice staff.
- Primary care networks and clinical directors.
- Healthcare professionals involved in medication management.
- Community pharmacists and support staff.
Qualifications and Experience
The successful candidate will possess:
- A minimum of 4-5 years of post-graduate experience, with some experience in acute hospital or primary care settings.
- At least 2 years of experience in a general practice environment.
- An Independent Prescriber qualification and extensive clinical knowledge in managing chronic conditions.
- Strong communication skills and a commitment to professional development.
