Position Overview

We are looking for a skilled and empathetic End-of-Life Care Nurse to become a vital part of our healthcare team. The ideal candidate will independently manage a diverse group of patients, create tailored care strategies, and collaborate with various external partners to ensure thorough and compassionate care. This position demands a high degree of clinical knowledge, sensitivity, and outstanding communication abilities.

Key Responsibilities

Patient Care Management:

  • Oversee a group of patients requiring end-of-life care, ensuring each individual receives customized and thorough support.
  • Perform comprehensive evaluations to identify the physical, emotional, and psychological needs of patients.
  • Track and assess patient progress, modifying care strategies as needed.

Care Strategy Development:

  • Formulate, execute, and assess personalized care strategies in partnership with patients, their families, and other healthcare professionals.
  • Ensure care strategies are based on evidence and tailored to meet the specific needs of each patient.
  • Provide education and guidance to patients and families regarding end-of-life care options and planning.

Independent Practice:

  • Operate autonomously to deliver exceptional end-of-life care, making clinical decisions within the defined scope of practice.
  • Utilize advanced clinical skills to manage complex symptoms and provide interventions aimed at enhancing quality of life.

Coordination and Communication:

  • Serve as a liaison among patients, families, primary care providers, specialists, and community resources.
  • Organize multidisciplinary team meetings to ensure cohesive and integrated care delivery.
  • Establish and maintain effective communication channels with external partners, including hospice services and support organizations.

Documentation and Reporting:

  • Keep accurate and current medical records, documenting all patient interactions and care provided.
  • Prepare and submit necessary reports and documentation promptly.

Professional Growth:

  • Stay updated with best practices and advancements in end-of-life care.
  • Engage in ongoing education and professional development opportunities.
  • Mentor and support junior staff and students as needed.

Quality Enhancement:

  • Contribute to the development and implementation of quality enhancement initiatives within the end-of-life care service.
  • Participate in audits and research activities to improve the quality of care provided.

Qualifications

Essential

  • Bachelor’s degree in nursing (BSN) or equivalent.

Desirable

  • Qualification in End-of-Life Care or a related field is preferred.
  • Independent Prescribing qualification is desirable.

Experience

Essential

  • Minimum of 3 years of experience in end-of-life care, hospice, or a related field.

Desirable

  • Experience in a primary care setting.

Skills

Essential

  • Advanced clinical assessment and decision-making skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a multidisciplinary team.
