Job Summary:

LOROS Hospice is seeking a highly motivated and experienced Health Care Assistant to join our Overnight Response Service team. As a Palliative Care Specialist, you will play a vital role in providing specialist advice and responsive visiting services to palliative care patients and carers overnight.

Key Responsibilities:

  • Work closely with a Clinical Nurse Specialist as part of a team of 2 to provide unplanned specialist advice and responsive visiting services.
  • Answer telephone calls and undertake home visits jointly with the specialist nurse.
  • Provide supportive patient care and input details onto SystmOne.
  • Contribute to the efficient running of the service by undertaking housekeeping duties.
  • Access to a pool car will be available, with driving being a key component of the role.

Requirements:

  • Experience of providing palliative care and end of life care within the community setting.
  • Empathetic communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Highly motivated individual with a strong commitment to delivering excellent patient care.
