Position Overview

Are you eager to engage with individuals in their residences and local communities, providing essential Occupational Therapy and Physiotherapy interventions?

An exciting role has become available for a Band 4 Community Therapy Support Practitioner within our dedicated service.

This position is ideal for candidates with prior experience as a therapy or rehabilitation assistant in healthcare or supportive environments.

You will join the Community Therapy Team, focusing on both clinic-based and home-based assessments and interventions aimed at preventing hospital admissions for older adults facing challenges.

The Community Therapy Support Practitioner will collaborate closely with therapy staff, medical professionals, and nursing teams to execute delegated responsibilities and explore interventions that empower patients to remain in their homes.

This role involves conducting positive risk assessments, modifying home environments, implementing therapeutic interventions, and coordinating with community teams for ongoing rehabilitation and larger environmental adjustments.

Previous experience in a community setting is preferred, but candidates with relevant training or work experience will also be considered.

Comprehensive support, training, and continuing professional development opportunities will be available to the successful candidate to enhance skills and competencies, facilitating improvements in practice and service delivery.

Main Responsibilities

Assist Occupational Therapists (OT) and Physiotherapists (PT) in delivering management and rehabilitation services to community patients.

Work collaboratively within a community-based multidisciplinary team to perform specific clinical and administrative tasks, implementing intervention programs as directed by the OT and PT, while adapting to the service user's needs.

Welsh language skills are desirable for this role; however, applications from both English and Welsh speakers are welcomed.

Support the Occupational Therapist and Physiotherapist by assisting with various therapeutic care modalities for patients in intermediate care and reablement.

Provide supervision and assessment of support worker competencies under the guidance of the Occupational Therapist and Physiotherapist.

Deliver care to patients as delegated by the Occupational Therapist and Physiotherapist, within the scope of your competencies.

Engage in project work, audits, and the provision of relevant statistics that may benefit patients or the Occupational Therapist and Physiotherapist.

Promote independence, health, and safety within the home and local environments.

Adopt a patient-centered approach to engage individuals in therapeutic activities, adjusting interventions based on their evolving needs.

Qualifications and Knowledge

Essential

  • Good general education, with English/Mathematics GCSE or equivalent qualification at grade C or above.
  • NVQ level 3 in a relevant subject or equivalent knowledge and experience, along with additional competency-based training.
  • In-depth understanding of rehabilitation interventions and their impact on patient abilities.

Experience

Essential

  • Experience working as an NHS band 3 OT, PT, or generic assistant.
  • Experience in public-facing roles within rehabilitation, recovery, or functional assessment.

Desirable

  • Experience in community settings.
  • Experience within NHS and/or Local Authority frameworks.
  • Prior experience with patients having diverse physical, psychological, emotional, and social needs.
