Job Overview

Are you interested in providing support to individuals in their residences and local communities through Occupational Therapy and Physiotherapy interventions?

A remarkable opportunity is available for a Band 4 Community Rehabilitation Support Practitioner within the @Home service.

This role is ideal for candidates with prior experience as a therapy or rehabilitation assistant in a healthcare or social care environment.

You will join the @Home Team located in a health park, delivering both clinic-based and home-based assessments and interventions aimed at preventing hospital admissions for older adults in need of support.

The Community Rehabilitation Support Practitioner will collaborate closely with the @Home Therapy team, Consultant Geriatricians, Advanced Nurse Practitioners, and Nursing staff to carry out assigned tasks and develop interventions that help patients remain in their homes.

Responsibilities will include conducting positive risk assessments, modifying home environments, implementing therapeutic interventions, and coordinating with community teams for ongoing rehabilitation and larger-scale adaptations.

Ideally, candidates will have demonstrated experience in a community setting, although those with relevant training or work experience will also be considered.

Comprehensive support, training, and continuing professional development opportunities will be provided to the successful candidate to enhance skills and competencies, facilitating improvements in practice and service delivery.

Key Responsibilities

Assist Occupational Therapists (OT) and Physiotherapists (PT) in delivering management and rehabilitation services to community patients.

Work collaboratively within a community-based multidisciplinary team to perform specific clinical and administrative tasks, implementing intervention programs in service users' homes and communities as directed by the OT and PT, while being responsive to the needs of the service.

Proficiency in Welsh is preferred for this position; however, both English and Welsh speakers are encouraged to apply.

Support the Occupational Therapist and Physiotherapist by assisting with various modalities of therapeutic care for patients requiring intermediate care and reablement.

Aid the Occupational Therapist and Physiotherapist in supervising and assessing the competencies of support workers.

Provide care to patients as delegated by the Occupational Therapist and Physiotherapist, within the scope of your competencies.

Engage in project work, audits, and the provision of relevant statistics that may benefit patients or the Occupational Therapist and Physiotherapist.

Promote independence, health, well-being, and safety within home and community settings.

Develop a patient-centered approach to engage individuals in routine therapeutic activities, adjusting clinical interventions based on their evolving needs.

Qualifications and Skills

Essential

  • Evidence of a solid general education, including English/Mathematics GCSE or equivalent at grade C or above.
  • NVQ level 3 in a relevant field or equivalent knowledge and experience with additional competency-based training.
  • In-depth understanding of rehabilitation interventions and their impact on patient abilities.

Experience

Essential

  • Experience working as an NHS band 3 OT, PT, or generic assistant.
  • Experience engaging with the public in rehabilitation, recovery, or functional assessment roles.

Desirable

  • Experience in community-based work.
  • Experience within the NHS and/or Local Authority.
  • Previous experience with patients having diverse physical, psychological, emotional, and social needs.
