Advanced Nurse Practitioner for Community Care

We are seeking a highly skilled and experienced Advanced Nurse Practitioner to join our team at Cwm Taf Morgannwg University Health Board. As a key member of our community care team, you will play a vital role in delivering high-quality patient care and contributing to the development of our services.
Key Responsibilities- Conduct holistic assessments of patient care needs and develop evidence-based programs of care.
- Exercise professional autonomy in relation to patient nursing needs and satisfy the expectations and demands of the role.
- Work closely with the team to provide effective leadership and management, ensuring the delivery of safe, effective, well-led, responsive, and quality patient care.
- Contribute to the development and implementation of policies and procedures within your sphere of responsibility.
- Participate in training and education programs, research, clinical audit, and service evaluation, demonstrating your ability to practice autonomously across all four pillars of advanced practice.
- Registered Nurse with a Master's award in Advanced Clinical Practice.
- Independent Prescriber with a working knowledge of vulnerable adult and child protection procedures.
- Experience in palliative care, chronic disease management, and wound management.
- Confident and competent in a wide range of clinical skills.
