Advanced Nurse Practitioner

We are seeking an experienced and skilled Advanced Nurse Practitioner to join our team in the Community Hospitals. The successful candidate will be responsible for providing high-quality patient care and working closely with the multidisciplinary team to ensure the delivery of safe and effective care.
About the RoleThe Advanced Nurse Practitioner will be responsible for the holistic assessment of patients' care needs and the development, implementation, and evaluation of evidence-based programs of care. They will work autonomously and as part of the team to maintain patients in a community setting and avoid hospital admissions.
The successful candidate will have a strong understanding of clinical governance and its application in practice, as well as a broad knowledge of health issues encompassing childhood and adulthood. They will also have experience in palliative care, chronic disease management, and wound management.
Key Responsibilities- Provide high-quality patient care and work closely with the multidisciplinary team
- Develop, implement, and evaluate evidence-based programs of care
