We are seeking a dedicated Clinical Nurse Leader for Adult Care to become part of a reputable and innovative provider of intricate care solutions within the community, focusing on adult care needs.

This role will be situated in a vibrant area where the organization is experiencing exciting growth and has secured additional care packages.


The successful Clinical Nurse Leader will take charge of delivering direct support to adult clients and their families, while developing customized care strategies tailored to each individual's requirements.

You will collaborate closely with a dedicated team of caregivers, mentoring and enhancing their clinical competencies and activities.


Essential Qualifications for a Clinical Nurse Leader:
Registered General Nurse (RGN) with a valid NMC PIN

Encouraging management approach that fosters team excellence

Recent experience in tracheostomy care and ventilator management within the last two years

Exceptional verbal and written communication skills to effectively engage with patients, families, and the multidisciplinary team (MDT)

Appropriate for Band 5 / Band 6 Nurses with proven leadership and management experience of Healthcare Assistants (HCAs)

Full UK Driving Licence is preferred but not mandatory

Key Responsibilities for a Clinical Nurse Leader:
Develop and implement a personalized care plan tailored to the unique needs of each client

Effectively coordinate the MDT through strong communication and collaboration

Provide ongoing evaluation, support, and assessment of clients' physical and psychological needs within your caseload

Administer nursing interventions, including the management of medication delivery

Establish and nurture meaningful relationships with clients' families

Collaborate closely with community teams, Continuing Healthcare (CHC) nurses, and Clinical Commissioning Group (CCG) commissioners to create and maintain individualized care packages

Competitive salary up to £45,000 per annum, depending on experience

1 day of remote work per week / 4 days in the community

Standard working hours from Monday to Friday (9 AM - 5 PM)
