Job Summary

We are seeking a skilled and compassionate Casual Rehabilitation Officer to join our team at North Lincolnshire Council. As a key member of our community rehabilitation team, you will play a vital role in supporting individuals with dementia to maintain their independence and live well in their own homes.

Key Responsibilities

  • Work collaboratively with individuals, their families, and healthcare professionals to develop and implement personalized support plans.
  • Provide practical assistance and guidance to help individuals maintain their daily living skills and well-being.
  • Conduct assessments and reviews to identify areas for improvement and develop strategies to address these.
  • Work autonomously and as part of a team to provide 24/7 support to individuals and their families.
  • Participate in training and professional development opportunities to enhance your skills and knowledge.

Requirements

  • Excellent communication and interpersonal skills.
  • Ability to work autonomously and as part of a team.
  • Strong problem-solving and decision-making skills.
  • Knowledge of dementia and its impact on individuals and families.
  • Ability to work in a fast-paced environment and prioritize tasks effectively.
