Job Summary:

The North Lincolnshire Home First Community Rehabilitation service is a registered Rehabilitation and Reablement service that provides time-limited intervention and therapies to individuals within their own homes. Our goal is to deliver support that puts people at the centre of our services, ensuring they can remain at home and feel confident and safe to live as independently as possible.

Key Responsibilities:

  • Support individuals with moving and handling, personal care needs, and administering medication.
  • Maximise opportunities for social inclusion and emotional wellbeing.
  • Work collaboratively with the transitions team to ensure smooth and effective transitions from hospital to rehabilitation and from the service for those with identified long-term care needs.

Requirements:

  • A previous qualification in care is desirable, though not essential, as full induction/training will be provided.
  • Ability to work flexible hours, including AM and PM shifts.
  • Opportunity to develop and apply for permanent positions within the Council.
