North Lincolnshire Council is seeking a dedicated Rehabilitation Support Worker to join our Home First Community support service.

This registered Rehabilitation and Reablement service provides time-limited intervention and therapies to individuals within their own homes. Our aim is to empower people to remain at home, feeling confident and safe to live as independently as possible.

Your Role
  • Provide rehabilitation support to individuals recovering from accidents or illnesses, assisting them in regaining independence.
  • Implement therapy plans and collaborate with other healthcare professionals to ensure comprehensive care.
  • Offer direct support, including assistance with moving and handling, personal care needs, and medication administration.
  • Create opportunities for social inclusion and emotional well-being, fostering a positive environment for recovery.
Essential Requirements
  • Flexibility to work weekends, unsocial hours, including bank holidays.
  • Ability to travel throughout North Lincolnshire.
