As a key member of our Home First Community support service, you will play a vital role in providing time-limited intervention and therapies to individuals in their own homes.

Key Responsibilities:

  • Deliver rehabilitation support to individuals following accident or illness, enabling them to regain independence and confidence.
  • Work collaboratively with other health and social care professionals to develop and implement therapy plans.
  • Provide direct support with moving and handling, personal care needs, and medication administration.
  • Maximize opportunities for social inclusion and emotional wellbeing.
  • Work flexibly, including weekends and unsocial hours, and travel around North Lincolnshire as required.
