Job Summary:

We are seeking a skilled and compassionate Hospital Rehabilitation Worker to join our team at North Lincolnshire Council. As a key member of our community support service, you will play a vital role in providing rehabilitation support to individuals following accident or illness, enabling them to regain independence and live safely in their own homes.

Key Responsibilities:

  • Deliver rehabilitation support to individuals in their own homes, following therapy plans and working collaboratively with other healthcare professionals.
  • Provide direct care, including support with moving and handling, personal care needs, and medication administration.
  • Conduct Trusted Assessments to identify and outline the level of care and support required, and implement accordingly.
  • Work flexibly, including weekends and unsocial hours, including bank holidays, and travel around North Lincolnshire as required.
