Job Title: Therapy Team Lead - Front Door Service

We are seeking a highly motivated and experienced Senior Physiotherapist or Occupational Therapist to lead our Front Door Therapy team at Lincoln Hospital. The ideal candidate will have a passion for delivering high-quality patient-centred care and a strong clinical background in physiotherapy or occupational therapy.

Job Summary:

The successful candidate will lead a developing and experienced clinical team of therapy staff of Bands 2-6 and will support the development of Therapy staff as part of the wider MDT to deliver evidence-based interventions in the front door and frailty specialty. The role requires the appointed person to be proactive and adaptable to support the smooth and prompt transition of hospital care to the individuals' place of discharge, taking into account their needs.

Key Responsibilities:
  • Lead the Front Door Therapy team to deliver high-quality patient-centred care
  • Develop and implement evidence-based interventions in the front door and frailty specialty
  • Support the development of Therapy staff as part of the wider MDT
  • Work closely with other members of the multidisciplinary team (MDT) to promote independence, provide rapid assessments, and facilitate safe, timely, and appropriate discharges
