Community Midwife for Vulnerable Women

Coventry, Coventry, United Kingdom University Hospitals Coventry and Warwickshire NHS Trust Full time

Position Overview

We are seeking a dedicated Midwife to join our community team on a part-time basis, focusing on delivering care to vulnerable populations. Integrated working arrangements can be discussed for those preferring full-time hours to enhance patient care continuity.

The successful candidate will be a registered midwife, contributing to the Lotus Team dedicated to supporting vulnerable patients. This team is committed to ensuring continuity of care for individuals facing social vulnerabilities, including mental health challenges, substance dependency, teenage pregnancies, refugees, and asylum seekers, while also working to prevent pre-term births. Opportunities for professional development and training in these specialized areas will be available for the right candidate.

The role requires the ability to work independently, manage a caseload effectively, and ensure appropriate referrals and safeguarding measures are in place.

The Midwife will operate in both hospital and community settings, collaborating closely with the Specialist Midwife for Perinatal Mental Health, the Substance and Alcohol Misuse Lead Midwife, and the Obstetric Lead for Perinatal Mental Health to address the unique needs of these vulnerable patients. Engagement with external organizations such as the Perinatal Mental Health team, Change Grow Live, and Family Nurse Partnership will also be essential. Responsibilities will include providing antenatal and postnatal care in clinics and patients' homes across the designated areas.

Main Responsibilities

  • Adhere to the Nursing and Midwifery Council's (NMC) code and Trust policies, serving as an effective role model in clinical settings.
  • Establish and monitor agreed standards of midwifery care, taking action to enhance quality where necessary.
  • Promote an evidence-based, sensitive, woman-centered approach to care in collaboration with patients and relevant healthcare professionals.
  • Exhibit strong verbal and written communication skills to ensure that professional advice and care instructions are clearly understood by patients, their families, and the multidisciplinary team.
  • Ensure continuity of care for patients and facilitate effective discharge planning to meet individual needs, involving the multi-professional team as required.

  • Collaborate effectively as part of the wider multi-professional team.
