Key Responsibilities

The successful candidate will collaborate with clinicians and staff across the East Warrington PCN as part of a multidisciplinary team. Responsibilities will encompass, but are not limited to:

  • Providing coordination and navigation for patients, working closely with social prescribing link workers, health and wellbeing coaches, and other primary care professionals.
  • Actively directing patients to the most suitable clinician, coordinating with clinical teams or external agencies to implement care plans as necessary.
  • Identifying patients for follow-up appointments and reviewing care plans.
  • Arranging appointments based on clinician directives, prioritizing urgent and non-urgent clinical needs.
  • Integrating a patient’s identified care and support needs into a personalized care plan, effectively communicating this to other healthcare professionals.
  • Assisting patients in accessing community services, peer support, or interventions that empower them to manage their health and wellbeing.
  • Ensuring patients receive high-quality information to facilitate informed choices about their care.
  • Collaborating with the PCN’s wellbeing team to enhance patient health and wellbeing.
  • Conducting patient visits in community, home, or care settings to assess and discuss care needs.
  • Utilizing population health data to proactively identify and engage with specific patient cohorts.
  • Supporting patients in utilizing decision aids for shared decision-making discussions.
  • Facilitating access to training, employment, and benefits through referrals to social prescribing link workers.
  • Building strong relationships with local community organizations to explore new opportunities.
  • Monitoring referrals to ensure timely and effective care delivery.
  • Participating in PCN meetings to provide updates on activities and initiatives.

About East Warrington PCN

East Warrington PCN is one of several networks within Warrington, Cheshire, providing Primary Care services to a significant patient population across multiple GP practices. With the introduction of enhanced services and additional role reimbursements, the PCN has expanded its team to improve patient care.

Candidate Profile

We are looking for a dynamic individual who can work independently or as part of a team. The role requires someone who is responsive, supportive, and a dedicated self-starter, committed to delivering high-quality service to patients.

The ideal candidate will possess excellent communication skills, empathy, and a solid understanding of primary care and community health services.
