Job Title: Clinical Care Coordinator

We are seeking an experienced Registered Nurse RGN to join our team as a Clinical Care Coordinator. As a Clinical Care Coordinator, you will be responsible for providing high-quality care to clients with complex needs in their own homes across the Nottinghamshire region.

Key Responsibilities:
  • Develop and implement individualized care plans to promote independence and quality of life for clients
  • Conduct regular visits to assess client needs and provide ongoing support and guidance
  • Collaborate with multidisciplinary teams to ensure seamless care delivery
  • Manage client case loads and prioritize tasks to ensure timely completion
Requirements:
  • Qualified RGN with a strong background in community health
  • Experience in developing care plans and managing complex care cases
  • Full UK driving license and car ownership
  • Strong clinical skills and ability to work independently
