About the Role

We are seeking a highly organized and detail-oriented Care Coordinator to join our team at PartnersHealth. As a Care Coordinator, you will play a vital role in supporting the delivery of high-quality patient care and services across our Primary Care Network.

Key Responsibilities
  • Run reports from clinical systems such as SystmOne to analyze appointment data and understand capacity and demand.
  • Book patient appointments and ensure accurate and up-to-date patient clinical records.
  • Work with teams within the GP practice and those who support GP practices, providing administrative support as required.
  • Generate reports to help analyze appointment data and identify areas for improvement.
  • Support patients to book appointments and manage their needs by answering queries, making and managing appointments, and providing clear and accessible information.
  • Support the coordination and delivery of multi-disciplinary meetings and clinics.
  • Adhere to organizational policies and procedures, including confidentiality and GDPR guidelines.
  • Work independently, plan own workload, and seek guidance as required from line manager and colleagues.
Requirements
  • Ability to work accurately and calmly under pressure in a busy environment.
  • Ability to keep accurate records and maintain confidentiality.
  • Administration and organizational skills to a high level, with the ability to manage workload, prioritize tasks effectively, and reprioritize in response to changing needs.
  • Integrity, sound professional judgment, and self-motivation.
  • Ability to work effectively under pressure, with hard work and reliability.
  • Problem-solving and analytical skills, with the ability to think outside the box.
  • Understanding of the importance of confidentiality and GDPR.
