About the Role

The Care Coordinator Specialist will work within the Nottingham City General Practice Alliance, providing coordination, administrative, and support to the clinical teams and other members of the organization. The primary responsibility of the post holder will be to coordinate the PCN response to the Directed Enhanced Service requirements across PCN GP practices.

Main Duties

The role will involve leading project work and collating practice data, identifying and understanding patient cohorts, patient engagement, and administrative tasks that support the smooth and effective running of the PCN. The administrative support to the primary care network will include activities such as organizing meetings, recording minutes, and action. The post holder will need to have excellent telephone communication skills to support patients and external providers such as care homes and safeguarding teams.

Key Responsibilities

  • Assess patient needs and help patients gain and use knowledge, skills, and confidence to become active participants in their care.
  • Act as a first point of contact for patients coming into the service and assess their engagement level.
  • Collaborate with the clinical teams and other members of the organization to provide coordination and administrative support.
  • Lead project work and collate practice data to support the smooth and effective running of the PCN.

Requirements

  • NVQ Level 3, advanced level, or equivalent qualifications or working towards or relevant work experience.
  • Excellent telephone communication skills and ability to work in a fast-paced environment.
  • Ability to work well under pressure and use own initiative.
  • Good understanding of healthcare and experience with SystmOne is desirable.
