About the Role

We are seeking a dedicated and compassionate Support Practitioner to join our team at Highland Home Carers. As a Support Practitioner, you will play a vital role in supporting individuals with learning disabilities in their own homes, providing 24/7 care and assistance with daily living tasks.

Key Responsibilities
  • Support and Assist: Provide personal care, assist with medication, meal preparation, and enable individuals to participate in various activities.
  • Work in Partnership: Collaborate with Assistant Service Managers, designated professionals, and other staff to develop and implement Personal Plans, ensuring individuals' physical, social, and emotional needs are met.
  • Financial Management: Ensure accurate handling of individuals' finances and maintain records in accordance with HHC policies and procedures.
  • Tenancy Support: Assist individuals in maintaining their tenancy, reporting any issues to relevant agencies, and maintaining outdoor spaces.
  • Confidentiality and Reporting: Respect confidentiality at all times, explain HHC policies to individuals and colleagues, and submit accurate daily reports and timesheets.
  • Flexibility and Teamwork: Be flexible, provide cover for colleagues, and undertake additional duties as required.
Requirements
  • Qualifications: SVQ Level 2 in Social Care or willingness to work towards this.
  • Communication Skills: Ability to communicate effectively with individuals, families, and health professionals.
  • Flexibility and Commitment: Willingness to work flexibly and commit to person-centred support.
