Clinical Nurse Specialist

We are seeking an experienced and skilled Clinical Nurse Specialist to join our team at Saint Francis Hospice. As a key member of our palliative care team, you will play a vital role in providing high-quality patient care and support to individuals and their families.
Key Responsibilities- Assessment and Care Planning
Conduct comprehensive assessments of patients' physical, emotional, and social needs, and develop personalized care plans to address these needs.
Telephone Support and EducationProvide ongoing telephone support and education to patients, families, and healthcare professionals, promoting self-management and empowerment.
Face-to-Face Assessments and InterventionsConduct face-to-face assessments and interventions in patients' homes, hospices, or other settings, as needed, to provide holistic care and support.
Collaboration and CommunicationWork closely with multidisciplinary teams, including healthcare professionals, social workers, and volunteers, to ensure seamless care coordination and communication.
Education and Quality ImprovementDevelop and deliver education programs for nursing and other disciplines, promoting evidence-based practice and quality improvement initiatives.
Leadership and ManagementTake on leadership and management responsibilities, including caseload management, team supervision, and quality assurance.
Professional DevelopmentEngage in ongoing professional development, staying up-to-date with the latest research, guidelines, and best practices in palliative care.
Requirements- Qualifications and Experience
Registered Nurse with a valid NMC registration and a minimum of 2 years' experience in palliative care.
Skills and CompetenciesExcellent communication and interpersonal skills, with the ability to work effectively in a team environment.
Knowledge and ExpertiseStrong knowledge of palliative care principles, practices, and guidelines, with expertise in symptom management, pain control, and end-of-life care.
