We are seeking an experienced Clinical Nurse Specialist to join our team at Saint Francis Hospice. As a Clinical Nurse Specialist, you will play a vital role in providing high-quality specialist support, advice, and symptom control to patients, their carers, and other healthcare professionals.
Key Responsibilities- Work collaboratively with the primary care team to provide specialist support, advice, and symptom control to patients, their carers, and other healthcare professionals.
- Provide specialist telephone assessment and support to healthcare professionals, patients, and relatives.
- Work autonomously and as part of a team to provide specialist resource in palliative care, collaborating with acute hospital and other specialist palliative care services.
- Provide specialist support, decision-making skills, expert advice, and education to members of the primary care team to support them in managing and caring for people with life-limiting illnesses in the community.
- Contribute to the development of specialist community palliative care services, aligning with the Palliative Care Strategy, Cancer Reform Strategy, and NICE Guidance.
- Undertake evidence-based, cost-effective non-medical prescribing decisions, ensuring safe prescribing practice within professional and organizational policies and guidelines.
- Excellent symptom control knowledge and understanding of current issues in palliative care.
- Evidence of recent and ongoing professional development in palliative care.
- Ability to work autonomously and as part of a team, with advanced communication skills and evidence of collaborative working.
- Ability to manage own work and work within a corporate caseload, with good IT skills and knowledge of electronic healthcare systems.
- Flexible approach to team working and an awareness of effective team dynamics.
- Good personal support mechanisms.
- RGN with significant post-registration experience.
- Teaching Qualification or equivalent level of experience.
- Degree or equivalent level of experience.
- Significant senior post-registration experience at a similar level in palliative care or another associated specialty.
- Non-Medical Prescriber or working towards.
