Patient Pathway Coordinator

An exciting opportunity has arisen to join the South Yorkshire & Bassetlaw Provider Collaboratives Commissioning Hub, hosted by Sheffield Children’s NHS Foundation Trust as the Patient Pathway Coordinator.
The post holder will be expected to be highly organised; with the ability to prioritise workload, work under pressure, and meet targets and deadlines. The individual should be self-motivated and driven whilst contributing to a high performing team.
The post holder will be a compassionate, accountable individual who treats all colleagues, patients and service users with respect and seeks to deliver excellence in their role. They will share the Trust values and be committed to equality, diversity and inclusion.
Working closely with the Case Managers and Clinical Director, to be a key contact for the SYB Commissioning Hub for all Single Point of Access and referral related matters.
To monitor and validate the received referrals from point of submission to admission or rejection.
To work as a member of the core Admin Team of the Commissioning Hub, provide cross cover and support as required in times of absence and leave.
Support the coordination of national and local audits by effective data collection by working closely and accurately with the Commissioning Hub’s Case Managers and Business and Performance Manager.
It’s a critical time to be joining the Commissioning Hub as we continue our journey to ensure quality of care is being provided as close to home as possible, in the least restrictive way and that we develop a range of preventative, community and bed-based pathways to support the SYB patient population.

Receive and process incoming referrals for inpatient admissions in a timely and accurate manner.
To be aware of the patients point in their pathway and escalate any issues that arise to the relevant Case Manager and Provider Collaborative Clinical Director.
To search for bed availability on all national units (not a clinical conversation).
Maintain an up-to-date and accurate record of patient tracking
The post-holder will participate in weekly bed management meetings as the key administration support. This will include preparation of reports and meeting templates.
Use the electronic patient database systems to input, validate and update patient information.
