The primary purpose of the Cancer Pathway Navigator role is a central point of contact for patients on a ‘rapid diagnostic, suspicious of cancer pathway’; helping to ensure the individual needs of the patient are supported and that care is built around those needs; as well as facilitating seamless, coordinated pathway delivery including through escalation to relevant departments; and a better, more personalised diagnostic experience for patients.

The post-holder will work closely with the wider cancer team (both clinical and administrative), as well as other key health and social care providers, to enable person centred care and efficient implementation of the pathway (including achievement of the 28 Day Faster Diagnosis Standard).

The Cancer Pathway Navigator will be involved from the beginning of the pathway and up to the point of definitive diagnosis (whether cancer or not). They will maintain effective communication both with and on behalf of the patient - signposting to other resources and information as defined by patient need; and liaising with staff and services to ensure tests and appointments are scheduled in a timely manner. It is envisaged that the Pathway Navigator will have a specific focus on the more direct support of harder to reach patient groups e.g. Learning Disabilities, ‘non-engagers’ etc.

The post holder will identify and resolve barriers to care, working flexibly and responsively to unlock bottlenecks and delays. Within the parameters of the role, the Pathway Navigator will help support the patient with shared decision making and self-management.

Respiratory Medicine is a single Directorate, with a presence on each hospital site and is a rapidly developing and growing speciality within the Sheffield Teaching Hospitals.

The Respiratory Medicine Directorate provides a comprehensive specialist Lung MDT service for the management of patients on a lung cancer pathway, including outpatient care and diagnostic services. A significant part of the Directorate’s capacity is needed to manage 2 week wait cancer patients. Currently there are 5 Lung Cancer consultants as well as a dedicated Lung CNS team within the Directorate. The Lung MDT administrative team provides a comprehensive service to patients, carers, internal and external users of the service, thus helping the Directorate to continue to develop an efficient patient led service.

This vacancy is advertised on a fixed term contract basis for a period of 12 months due to the post being externally funded. The fixed term contract status of the post holder will be reviewed in accordance with the Trust's legal obligations regarding fixed term contracts taking into consideration the ongoing source of the external funding and the requirement for the post to be maintained.
