Job Description: Patient Care and Transportation Coordinator

As a Patient Care and Transportation Coordinator, you will be responsible for transporting patients to and from various locations, ensuring their safe and timely arrival at their destinations. This role requires strong communication skills, attention to detail, and the ability to work independently.

You will work closely with our team to develop and implement effective transportation strategies that meet the needs of our patients and partners. Your responsibilities will include:

  • Transporting patients to and from hospitals, secure mental health facilities, prisons, police stations, and other psychiatric settings.
  • Maintaining accurate records of patient transportation, including details of destinations, times, and any incidents that may occur during transportation.
  • Ensuring vehicles are properly maintained and equipped to meet the needs of patients.
  • Developing and maintaining relationships with healthcare professionals, patients, and their families to ensure seamless care coordination.
