An essential role in providing a critical and fluid-structure to our Carers on a daily and weekly basis. With a focus on relationships and service delivery, this role will enhance the quality of care provided to our clients by supporting all our Carers to achieve their visits in a timely and consistent way. With attention to varying demands including local areas, business costs and skill set - this role will require extensive experience which will equate to a Supervisory level.

**Principle Responsibilities**:

- Be responsible for the scheduling of our Home Care Teams across the area and manage communication on a daily basis ensuring all calls across the area are covered and attended;
- Support team members across the area and where possible ensure all schedules are covered by matching Carers with clients’ needs while meeting their preferences in regards to days, times and duration of visits;
- Lead by example ensuring that any reasonable requests are acted upon and any concerns or complaints are promptly investigated and dealt with appropriately;
- Planning care rounds whilst focusing on Carers' locations and travelling times when completing the schedule. Awareness on how a more efficient care round can benefit both our Home Care Team and the needs of the business;
- Show commitment to your team by supporting new initiatives and making recommendations for continuous improvement;
- Deal with daily enquiries from new and existing clients and signpost or provide information about other professional services that may be required;
- Demonstrate a collaborative mindset towards your team and Registered Branch Manager by demonstrating accountability at all times;
- Communicate effectively with the Registered Branch Manager, Deputy Manager, Supervisors and other professionals with regards to clients and Carers with a focus on actioning responsibly and managing all matters confidentially;
- Ensure all Carers know how to use our care planning software in their day to day roles. Encourage and help towards full compliance;
- Manage alerts on the system to ensure that they are actioned;
- Record staff sickness, absence & holidays;
- Responsibility for the emergency on-call phone on a rota basis as instructed by the Registered Branch Manager and understand this may change and is not a constant structure;
- Cover care calls when necessary in cases of urgent need;
- To assist the Registered Branch Manager, Deputy Manager and Supervisors in being a primary communicator between Clients and Carers;
- To primarily support the Registered Branch Manager by learning and adopting appropriate responses and actions when liaising with queries from Clients and Carers and liaising with the appropriate external professionals, agencies and families;
- Accountability of organising and supporting work-based learning for newly appointed Carers to ensure sufficient support;
- To attend, shadow and learn how to arrange and undertake regular supervision with Carers;
- To attend any training courses as deemed beneficial to your job role; and,
- To ensure all communications are logged in the appropriate communication records

**Essential Criteria**:

- Minimum of 3 years experience in the Care Sector;
- Previous Care Coordinator experience desired
- Good standard of general education;
- Availability to cover all operational hours during the on-call week and on occasion, outside and including office hours on an ad-hoc basis;
- A desire to work towards NVQ (QCF) Level II or III; and,
- A Social Care Portfolio (of courses attended etc.).

**Desired Qualifications**:

- NVQ / QCF / RQF II or lll in Health & Social Care.

**Person specification**:

- Strong communication & organisational skills;
- Self-reliance and self-motivation with a collaborative team-orientated approach to all aspects of work;
- Ability to be resourceful, proactive and accountable when issues arise;
- Experience in coordinating care schedules;
- Excellent IT skills; and,
- A clean driving licence with access to your own car.
