1 x Patient Care Coordinator (Full Time) Monday - Friday 8am - 4pm

Due to practice growth an exciting opportunity has arisen to join our excellent administrative team at Princes Gardens Surgery, a forward-thinking and respected practice in Aldershot, Hampshire.

We are looking for a dynamic and enthusiastic individual to join our dedicated team as a Patient Care Coordinator. You will be a core member of the administrative/reception team but will build and learn enhanced care coordination skills. The successful applicant will have exceptional people and communication skills. The ability to problem solve and use initiative is key. Working under the direction of the GP’s, management and strictly in accordance with practice guidelines and protocols.

The Patient Care Coordinator will facilitate the patient journey in accessing the appropriate service or healthcare professional in a courteous, efficient, and effective way. The role involves dealing with patients on the front line, taking history and guiding the patient to the most appropriate service effectively. The ability to assess the difference between desire and need is a must, providing the best options and reaching a positive outcome for the patient while projecting a positive and friendly image, either in person or via the telephone.

You will need to demonstrate excellent interpersonal and communication skills. You must be able to prioritise tasks and manage time effectively, also be able to work in an integrated manner and flexibly within the team and be confident in completing individual tasks. The role holder will mentor, train and support Reception Staff in their patient signposting roles.
