We are currently seeking an Admissions and Discharge Co-ordinator to join our supportive team at the highly specialist and well sought-after Blackheath Brain Injury Rehabilitation Unit in London.

Located in trendy South East London, 10 minutes-walk away from the pretty cafes and boutiques of Greenwich, the unit has a 19-bed Level 1 Highly Specialist Cognitive-Behavioural ward with and a Level 2 Neurorehabilitation ward with 18 beds. Both wards provide specialist treatment to patients with physical and cognitive impairments resulting from an acquired / traumatic brain injury.

You will be an integral part of an enthusiastic, supportive, and highly experienced multidisciplinary team (neurology, neuropsychiatry, neuropsychology, nursing, physiotherapy, speech and language therapy, occupational therapy, social workers) working proactively across Blackheath ensuring that all plans and arrangements are in place for patients’ referrals, admission, and discharge.

The Blackheath Brain Injury Unit is a renowned centre for complex neurorehabilitation and is part of the Active Care Group Neuro Network (consisting of 25+ sites nationally) which has a focus on the use of clinical expertise, innovation, and technology to meet patients’ expectations of their rehabilitation goals.

There is a strong ethos of patient-centred high-quality care which drives clinical and administrative decision-making. There have been exciting new renovations to the wards and value is placed on staff wellbeing.

**For this role you will need to hold a UK professional health qualification, which requires you to be HCPC, NMC or Social Worker Registered.**

**What you'll be working**:
This role is 37.5 hours per week, Monday to Friday

**What you'll be doing**:

- Work closely with the MDT to ensure maximum bed occupancy and ensure the appropriateness and timeliness of all referrals
- Update the badgenet system in a timely fashion
- Lead person on the weekly meetings on referrals, admissions, discharge and transfers
- Have a key role in co-coordinating all inpatient admissions and discharges, ensuring that all is in line with national admission, discharge policies and processes
- Co-ordinate all appropriate procedures working with the doctors, nurses, therapists, Family, Multi-Disciplinary Team (MDT) and outside agencies ensuring that all plans are in place for the time of admission
- Be proactive on each admission, introducing themselves and their role to all patients and carers
- Ensure all parties understand when support will be required to facilitate discharge, ensuring that any carer(s) of the patient are identified and supported through the discharge process
- Ensure timely referral to other services the patient is likely to require after discharge: ongoing health, housing or social care
- Keep people safe from harm and protect their human rights

**What you'll have**:

- HCPC, NMC or Social Worker Registered
- Analytical and judgement skills that will be applied in the interpretation of complex assessment data, to be able to make appropriate decisions in relation to admitting and discharging patients
- Capacity to summarise and interpret highly complex and sensitive data and write concise, accurate and readily understood reports that communicate the findings of the neuropsychological assessment to the patient and their family, the MDT and external stakeholders
- Developing and maintaining rapport with people who may have emotional, behavioural and cognitive impairments
- Ability to Interact with and understand families who are under acute or chronic stress
- Excellent IT skills
- Excellent, professional literacy skills
