The Homelessness Pathway Team is a new service that works across the Newham University Hospital in-patient and Accident and Emergency Services to provide advocacy, support and quality discharge interventions for homeless clients attending or admitted to Newham University Hospital. The Homelessness Pathway Team works in close collaboration with the wider hospital discharge teams.
The dual aims of the Homelessness Pathway Team are to improve the quality of care for homeless patients, whilst reducing potential delayed and also premature discharges. There is an overarching aim to reduce future unscheduled admissions and A&E attendances.

This is an exciting post to improve the care for homeless people admitted to Newham University Hospital. The post holder will work alongside a specialist practitioners, a social worker & other specialist workers to provide additional support to homeless people. The post holder will be responsible for the identification of homeless people admitted to the hospital and will work with the specialist GP and Homeless Team to provide additional care for those patients. The post holder is responsible for linking the person to the right services to meet their needs e.g 3rd sector, primary care, housing, Mental Health, drug and alcohol services etc and will have responsibility for overseeing homeless step-down capacity and managing the flow for Newham.
The post holder will be responsible for the enhanced hospital management of homeless people referred to the team or identified by the team. The post holder will provide direct support to medical & surgical teams, therapists & the Ward Sisters in discharging their patients in a safe and timely manner.

The post holder will work closely with other members of the Homeless Team, specifically the Clinical Leads who will provide medical support to homeless people. The Homelessness Pathway Team will work closely with staff across all disciplines between hospital and community to ensure a safe, timely and effective transfer of patient care on discharge. The Homelessness Pathway Team aims to improve the health of homeless people, provide holistic co-ordinated care.

The Homelessness Pathway Team staff team is a multi-disciplinary and multi-agency team, and is currently in development. As this is a ‘pilot’ project you will need to
- Be responsible for data collection to inform performance of the pathway
- Be responsible for implementing a system to collect feedback from people who use the pathway
- Track outcomes and support evaluation.
