An exciting opportunity has arisen for a dedicated and passionate professional to join the KHP Pathway Homeless Team in Kings College Hospital.

We are looking for a highly motivated individual with strong commitment to supporting homeless patients in a hospital setting. The key aim will be to ensure safe, appropriate, effective and sustainable discharge for patients transitioning from hospital care to the communities. The role includes the potential to work across all KHP Homeless Team sites and incorporates flexible working hours, such as rotational weekend working.

The post holder will work in partnership with other members of the multi-disciplinary team, as well as other hospital teams and outside agencies in order to achieve successful discharges. The post holder will work both on the wards, and in A&E, and may need to outreach into the community to support patients post discharge.

An enhanced DBS check is required for this post.
- Deliver individualised advice to patients, liaising with the necessary support agencies/authorities to achieve the best possible outcome on discharge
- Act in an advocacy role both within the clinical setting and with external agencies using the expertise of peers in a multidisciplinary team as appropriate.
- Ensure patient case notes in EPR (Electronic Patients Record) and Emis are updated in a timely manner
- Carry out detailed housing assessments to help prove a patient’s homelessness, eligibility, priority for housing and local connection
- Joint working with British Redcross to facilitate a patients transition from Hospital care to the community ensuring safe handover where required.

The KHP Homeless team works across the Kings Health Partner Inpatient and Accident Emergency Services to provide advocacy, support and quality discharge interventions for homeless clients attending or admitted to any of the Kings Health Partner hospitals. The Homeless Team works in close collaboration with the wider hospital, Inclusive of discharge teams, local authorities (Housing and Social Care), community healthcare services and third sector housing charities.. The team currently operates within the St. Thomas’, Guys, Kings College, and Maudsley & Lambeth Hospitals. Guys and St. Thomas’ NHS Foundation Trust is the employer for all the teams. The dual aims of the Homeless Team are to improve the quality of care for homeless patients, whilst reducing potential delays and premature discharges. Itis also overarching aim of the team to reduce unscheduled admissions and A&E attendances, with a keen focus on reducing health inequalities for homeless patients

The post holder will ensure safe, appropriate, effective and sustainable discharges that meet the needs of homeless patients, supporting a smooth transition from hospital care to the community. The post holder will work in partnership with members of their own multi-disciplinary as well as other hospital teams and outside agencies in order to achieve successful discharges. The post holder will work both on various wards, in Accident and Emergency (A&E) and may need to outreach into the community to support patients post discharge for follow ups.

The post holder will work directly with a nominated Band 6 or 7 Nurse, Occupational Therapist or Social Worker in the team, and will build relationships with local housing authorities.

The post holder will provide housing advice to other members of the team. The post holder will undertake detailed housing assessments that will help prove a patient’s homelessness, eligibility, priority for housing and established local connection. The post holder will escort patients to Housing Options services to provide advocacy as necessary and will liaise with Local Authorities Housing Departments to achieve sustainable housing discharge outcomes where possible.

The post holder will also provide discharge advice and support to other health professionals in the hospital as necessary. The post holder will also be expected to contribute to teaching/awareness sessions delivered to a variety of staff throughout the hospitals.

The post holder will be engaged in significant partnership working aimed at improving hospital discharge for individual homeless patients where patterns emerge. This partnership working involves key stakeholders NHS, Housing, Social Care and Voluntary Sector organisations.

The post holder will uphold and adhere to Trust clinical guidelines on practice and will endeavour to communicate the Trust’s vision at all times.
