**We'd like you to join us as an **Admission and Discharge Co-ordinator - Registered Nurse, **at our Blackheath Brain Injury Rehabilitation Centre in South East London.**

At Blackheath Brain Injury Rehabilitation Centre, we are specialists in the treatment of patients over the age of 18 with physical and cognitive impairments and/or neuropsychiatric disorders resulting from a brain injury or other neurological condition. We support our patients through assessment, rehabilitation, support and care across two wards.

This regional service is for people with complex cognitive and /or behavioural problems following a brain injury or as a result of a neurological condition. They have complex nursing and therapy needs requiring input from a highly specialist Inter-Disciplinary Team, and treatment facilities unlikely to be available locally.

This service is suitable for people who require specialist inpatient rehabilitation and have ongoing physical therapy needs.

**What you'll be working**:

- Monday to Friday
- 9.00am - 17.00pm

**What you'll be doing**:

- The admission and safe discharges of inpatients and supporting the MDT with more complex discharges across the service.
- The post holder will have responsibility for the referral pathway and ensure all referrals on badgenet system are managed in line with the NHSE specifications.
- Lead person on the weekly meetings on referrals, admissions, discharge and transfers.
- Playing a key role in co-coordinating all inpatient admissions and discharges, ensuring that all is in line with national admission, discharge policies and processes.
- Co-ordinate all appropriate procedures working with the doctors, nurses, therapists, Family, Multi-Disciplinary Team (MDT) and outside agencies ensuring that all plans are in place for the time of admission.
- Communicate with patients, their families and the person who referred this patient. To verify all parties understand admission plans, dealing with their concerns in a compassionate and effective manner.
- The discharge coordinator will ensure that any carer(s) of the patient are identified and supported through the discharge process. This includes providing information on Carer’s Assessments and support services and/or referrals to the relevant support services. Ensuring the carer has adequate support in place will reduce the risk of unnecessary readmission of the patient.
- The discharge co-ordinator will attend weekly ward rounds and input the discharge planning for patients on the electronic care system to inform decisions and plans.
- To maintain close working relationships with the neuro navigator especially around complex discharges.
- Liaise with all relevant internal and external colleagues including Clinical Leads, CQC, Safeguarding Team, NHS England, Clinical Commissioning Groups (ICB).
- To develop relationships, ensuring patients and carers are involved in all aspects of care. To be accessible and approachable for patients / relatives.

**What you'll have**:

- A current NMC Pin number RNA
- A Legal Right to work in the UK
- Analytical and judgement skills that will be applied in the interpretation of complex assessment data, to be able to make appropriate decisions in relation to admitting and discharging patients
- Capacity to summarise and interpret highly complex and sensitive data and write concise, accurate and readily understood reports that communicate the findings of the neuropsychological assessment to the patient and their family, the MDT and external stakeholders
- Developing and maintaining rapport with people who may have emotional, behavioural and cognitive impairments
- Ability to Interact with and understand families who are under acute or chronic stress
- Excellent IT skills
- Excellent, professional literacy skills
