Essential responsibilities will be agreed at an annual review of the job plan in accordance with Trust guidance for job planning for advanced and consultant practitioners which will ensure the tACP is able to meet the required pillars of advanced practice (NHS England 2019), enabling services to be designed and developed within the context of best use of resources, required service capacity and the individual professional development needs of the tACP. To be professionally and legally responsible and accountable for all aspects of own work, maintaining a high standard of clinical care and enhancing the patients experience within Oncology & Haematology. Uses specialist knowledge and specific training as an tACP to act as part of the MDT in the management of patients referred to the service, providing assessment, planning, and implementing a comprehensive management plan within a defined and approved scope of practice. This includes ordering and interpreting appropriate investigations where appropriate and assimilation of highly complex information to reach a potential differential diagnosis.

The tACP, alongside the MDT, will determine a treatment/management plan in line with the diagnosis and any test results following local and national guidelines. Access Trust electronic/patient administration systems/ICE and paper systems for relevant clinical information, to assist with clinical prioritisation and patient management. This may include requesting, reviewing, interpreting investigations, chasing results, and reporting on any findings to relevant parties, according to agreed scope of practice. Develop extended investigative and advanced analytical skills to elicit a comprehensive assessment of patient history appropriate to the clinical situation, which will include presenting complaint, history of the present illness, past medical history, social history, family history, medications, and allergies, review of systems, risk factors and appropriate targeted history.
