We have an exciting opportunity within the Urgent Community Response Team in Lincolnshire for a Therapy Assistant to join our team. We are a supportive and fun team who are passionate about supporting patients to remain within their own home and prevent unnecessary admission to hospital. We are a team consisting of Nurses, Paramedics, Physiotherapists, Occupational Therapists and Support staff.

The Urgent Community Response Team is a service that is open 7 days a week 365 days a year from 8am - 8pm. Shift patterns available are for early, late or long shifts and to work flexibly).

The UCR service works with patients within their own home to prevent unnecessary admission to hospital. We do this by carrying out a holistic assessment, providing support, advice, provision of equipment and on ward referrals to other services. We work closely with patients, their families and carers and various other professionals and services within Lincolnshire.

The team has various bases across Lincolnshire to work from.

To provide an effective evidence based therapeutic service for
patients referred to the Urgent Community Response Team in
assessments and specialist interventions.

To use your personal effectiveness and knowledge to deliver a
quality service.

Under direction of registered practitioner develop and implement
an individual plan of care through assessment of the immediate
and ongoing needs of the patient, in accordance with Trust
policies, procedures, protocols and guidelines.

To implement, monitor and progress therapy interventions for
patients assigned to you in accordance with agreed protocols.

To communicate effectively with other health care professionals
and outside agencies regarding the patients care.

To maintain comprehensive documentation and patient records.

To prioritise own work, balancing patient related and non clinical
demands and ensure that these remain in accordance with those
of the service.

To be responsible for the safe use of all equipment within their
scope of practice.

Assist practitioners in delivering complex treatment interventions.

To contribute to inter-agency/multi-disciplinary team working
